The Social Worker is responsible for assuring department and all individuals at their direction provide intake and patient advocacy services as well as coordinating social service matters with physicians, staff, patients, families, and other organizations.
He/She will strive to lessen or remove blocks to the patient's ability to access wellness and the use of medical/rehabilitative care across the continuum of care.
These activities include serving as a resource to other care providers regarding the following:
1) accessing community resources, 2) arranging for needed medical equipment or supplies to assist in returning client to their home, 3) appropriate referrals to state and government agencies when issues identified, 4) referrals to community self help organizations, 5) serving as a representative at meetings that integrate our ADHC with other health, education, and welfare agencies of the community, 6) providing individual and group counseling as defined within the participant's Individual Plan of Care (IPC).
This person will also provide indirect supervision of Program Aides.
The Social Worker will work collaboratively with a team of licensed professionals, including Occupational Therapist, Speech Therapist, Physical Therapist, RN's, Dieticians, and Physicians to coordinate Social Service planning and services that assists participants and their families in understanding and coping with emotional and social programs that affect their health status.
The Social Worker will report to the Program Director.
Primary
Responsibilities:
Provision of ADHC social services, including various aspects of assessment (initial assessment, quarterly note, periodic assessment, annual assessment, etc.
).
Care plan development and advocacy for each participant.
Evaluate patients; identify limitations which impede their daily living, monitor and report case recording on timely basis, participate in care plan development and coordination of social/health services.
Research and coordinate community referrals.
Conduct home assessments and outreach.
Participate in interdisciplinary team meetings and provide consultation to therapists, families, and patients.
Assist the Program Director in day-to-day management of the Social Work Department, supervision of staff, audit preparations, and reporting.

Qualifications:
Master's Degree in Social Worker highly preferred.
Previous experience in various aspects of social service.
Excellent communication skills; written and verbal.
Ability to provide individual and group counseling.
Experience in working with case management of the elderly and multi-disciplinary team environments.
Knowledgeable in community resources.
Basic Computer Skills; PC, Microsoft Office, Internet.
Must have reliable transportation and auto insurance.

The check will bounce and the victim is left responsible.
